Step 2: Fill Out Required Forms
All New Patients Will Need to Complete Online Forms
Instead of filling out forms in the office, we have electronic versions of all our required forms that you can fill out prior to your visit. We hope this will make it much easier and safer for us all.
VERY IMPORTANT! You will receive an email with a special link to fill out your patient intake forms. Be sure to check all your email folders after you have made your appointment for your patient forms link.
Filling Out Insurance Information
When filling out the insurance information form, please be thorough. Provide accurate information on your current dental plan only (we do not bill medical). If you are unsure of your dental carrier, please contact your HR department for the information.
Delta Dental subscribers please include the state for which your dental claims are processed. This information can be found on the back of your dental insurance card in the claims mailing address. If you do not have a card, then enter the state for which your employer is headquartered.
Insurance information is obtained through either your member ID# or subscriber’s social security number and date of birth. This information is pertinent for verifying insurance coverage, obtaining a summary of your dental benefits and sending insurance claims.

Step 4: Prepare for Your Appointment
What To Expect
Our new patient Comprehensive Exam is a two-hour appointment.
Your initial visit will typically not involve a cleaning. During this visit, we assess your oral health, perform an iTero scan, and make any needed diagnosis before administering treatment. One of our experienced dental assistants will begin by recording all necessary documents to establish care in our office. We will take digital X-rays, intra-oral images, and a digital scan.
We will review your health and dental history and address any concerns you may have. A Registered Dental Hygienist will evaluate your gums to determine the type of cleaning needed at a future appointment. You will then be introduced to one of our amazing Doctors. Dr. Andy or Dr. Chris will complete the exam and provide a comprehensive dental health diagnosis.

What About Insurance?
While we accept most insurance plans, Wolken Family Dental is considered an out-of-network provider—meaning your insurance policy may not guarantee dental insurance benefits. If it doesn’t, you’ll be responsible for paying anything your insurance doesn’t reimburse, even if it’s more than the initial estimate you received. Depending on your plan, you may also need to pay in full at the time of service, then get reimbursed directly by your insurance provider.
